I only use the information collected to provide my services. I do not share or sell the information you give me for any other purpose than providing the service you require from me or for insurance purposes.  At any time you may request a copy of the information I have recorded about you. You may also request that all information with respect to yourself as a client be permanently deleted.

I request the minimum level of personal identification information required to run my business. This data is provided to me by you directly or by you filling in the forms required during online booking. I will never obtain information about you indirectly from any other sources. The information I keep on records are your full name, address, date of birth, phone number and email, and medical records and I will also keep a note of every appointment for future reference.

 

I consider you have given consent for me to store your information based on your receiving services from me.

You may receive marketing emails related to the treatments you may have had in the past. I also use the marketing email to let my clients know of any important information about the business.
